Uterine fibroids

Uterine fibroids are a common condition that affects many women, often causing a range of symptoms, such as pelvic pain, heavy menstrual bleeding, and pressure on surrounding organs. While they can vary in size and location within the uterus, the impact on a woman’s health can be significant depending on the fibroid’s characteristics and size.

Common Symptoms

Sharp and acute pain in the abdomen

Acute pelvic pain on fibroid site

Less severe, lasting pelvic pain

Heavy bleeding

Fever

Temporary white blood cell count elevation

Risk Factors

  • Age: Most common in women in their 30s and 40s, particularly as they approach menopause.
  • Family history: Having a close family member with fibroids increases the risk.
  • Ethnicity: African American women tend to have higher rates of fibroids.
  • Hormonal imbalances: High estrogen and progesterone levels can promote fibroid growth.

Limitations of Medication Therapy

  1. Hormonal Treatments (e.g., birth control pills, GnRH agonists):
    • Limitation: These help manage symptoms like heavy bleeding but do not shrink the fibroids long-term. Side effects like mood swings, weight gain, and bone thinning can also occur with GnRH agonists.
    • Not ideal for: Women who want to preserve fertility because these medications can suppress ovulation and menstruation.
  2. Myomectomy (removal of fibroids):
    • Limitation: A major surgery with a significant recovery period. Though it preserves the uterus, fibroids may return over time, requiring further procedures.
    • Not ideal for: Women with multiple or large fibroids.
  3. Endometrial Ablation (removal of the uterine lining):
    • Limitation: Effective for controlling heavy bleeding but does not treat the fibroids themselves. It is also not an option for women who wish to maintain fertility.
    • Not ideal for: Women who still wish to conceive.
  4. Uterine Artery Embolization (UAE):
    • Limitation: This procedure reduces blood supply to fibroids, causing them to shrink. However, it may cause severe pelvic pain, risk of infection, or early menopause, and fibroids may regrow.
